5C Self-Assessment

The 5C Leadership Performance System

How to Use This Assessment

Rate each statement from 1 to 10 based on how true it is for you when under pressure — not on your best day.

1-3: Costing me (shows up regularly under pressure)

4-6: Inconsistent (have it on good days, slips under real pressure)

7-10: Reliable (have tools, use them, holds up even on worst days)

Consciousness

Awareness of self, others, and the environment. The foundation for intentional action and mindful leadership.

I notice my state (stress, fatigue, reactivity) before it drives my behavior.
When something doesn't go as planned, I separate the facts from the story I'm telling myself about them.
I catch my own patterns (overcommitting, avoiding, controlling) in real time, not in hindsight.

Connection

Building meaningful relationships that foster trust, empathy, and collaboration.

I make a conscious effort to build strong relationships with the people I work with.
I have a good sense of my colleagues as people, not just coworkers.
I feel positively connected to the work I'm doing and where I'm at in my career.

Communication

Clear, authentic expression that ensures alignment, reduces friction, and amplifies impact.

I say the hard thing early, clearly, and cleanly.
Under pressure, people know what I'm thinking and what they can count on from me. No guessing.
I listen to understand, not just to respond.

Consistency

Reliable actions and behaviors that build credibility and long-term trust.

I am the same professional on a bad day as on a good one.
People know what to expect from me, regardless of my mood or workload.
I follow through on what I said I'd do, even when it's inconvenient.

Confidence

The belief in one's abilities and decisions that inspires others and drives decisive action.

I trust myself under pressure because I have tools and I use them.
I make the call when it's mine to make instead of stalling for certainty.
I can hear hard feedback without collapsing or counterattacking.
